Report an Incident

While our ultimate goal is a unified and inclusive campus environment, incidents of bias and hate do occur. They might involve students, faculty, staff, administrators and other community members. Report incidents of bias or hate by submitting a Bias and Hate Incident Form.

Submit a Bias and Hate Incident Form 

If you feel that you've been a victim of a hateful act, please submit a Bias and Hate Incident Form so that we are aware of such incidents and, when necessary, we can enforce appropriate consequences.  We will respond back within 72 hours after submission.


How to Identify a Bias or Hate Incident

A bias incident is a non-criminal act that is motivated in whole, or in part, by a victim’s actual or perceived sex, race, ethnicity, religion, gender, gender identity or disability. This type of incident may be directed toward and individual or group.

Bias and hate incidents contribute to creating an unsafe or unwelcoming environment.  Not all bias incidents are considered discrimination or harassment under the University of South Carolina’s Non-Discrimination Policy [pdf].


Submitting Information Anonymously

A bias incident report can be completed anonymously. However, if you want the Office of Diversity, Equity and Inclusion or the Office of Equal Opportunity Programs to follow up with you regarding the incident, you will need to provide us with appropriate contact information.

Reports that are submitted anonymously will still be investigated, but only if the report includes enough factual information upon which to further investigate. Even if you submit a report anonymously, we may contact you if you are identified through the investigation, and NOT because you submitted the anonymous bias report.

We encourage anyone who has experienced, witnessed or learned of a bias or hate incident to complete this form and we will respond back within 72 hours after submission. You may complete as much or as little information as you choose.
